Mail In
Complete a registration form and mail the form with payment to:

Check first and second class choices.
Please do not mail cash.

Drop Off Box
Place registration form in a sealed envelope and drop off in designated box at the Pavilion, 1000 Wellington Avenue, Elk Grove Village.

In Person
Visit the Customer Service Desk at either of two locations during business hours:

Mon. - Fri. 9:00 am - 7:30 pm
Sat 9:00 am - 1:00 pm

Al Hattendorf Center, 225 East Elk Grove Blvd, Elk Grove Village

Checks:   Checks may be used with the Mail In, Drop Off Box, or In Person methods only. A $25 fee will be assessed for insufficient funds checks returned.
